Services Provided

LabCheck Diagnostics, LLC provides diagnostic testing, screening, specimen collection, and related services, including but not limited to:

Clinical and non-clinical testing

  • Employer and DOT-related testing

  • Screening and wellness services

  • Educational or certification services (such as BLS classes, when offered)

  • All services are provided by appointment unless otherwise stated.

 

Eligibility & Information Accuracy


By using our services, you confirm that:
 

  • You are at least 18 years old, or a parent/legal guardian consenting on behalf of a minor

  • All information provided is accurate and complete

  • You have legal authority to consent to testing when required

 

Providing false or misleading information may result in denial of services.

 

Appointments, Payments & Cancellations

 

Appointments

 

Appointments made in advance are preferred, but walk-ins are accepted.

 

Payments
 

Payment may be required at the time of booking or at the time services are rendered. Accepted payment methods will be disclosed during scheduling.

 

Cancellations & No-Shows

  • A minimum of 24 hours’ notice is required for cancellations or rescheduling

  • Late cancellations or missed appointments may result in a fee

  • Prepaid services may be forfeited if cancellation policies are not followed

 

Testing Results & Turnaround Times

  • Test result turnaround times are estimates and not guaranteed

  • Delays caused by third-party laboratories, couriers, or circumstances beyond our control are not the responsibility of LabCheck Diagnostics, LLC.

  • Results are released only to authorized parties in accordance with applicable laws and privacy regulations

 

Medical Disclaimer
LabCheck Diagnostics does not provide medical diagnosis, treatment, or medical advice unless explicitly stated.
 

  • Test results should be reviewed with a licensed healthcare provider

  • LabCheck is not responsible for medical decisions made based on test results

  • Our services do not replace professional medical care

 

Privacy & Confidentiality
Your privacy is important to us. All personal and testing information is handled in accordance with our Privacy Policy.

 

By using our services, you consent to the collection and use of information as outlined in our Privacy Policy.

Termination of Services
 

LabCheck Diagnostics reserves the right to refuse or terminate services at our discretion for reasons including but not limited to:

Termination does not eliminate payment obligations already incurred.
